The sysmouse support dose not work on FreeBSD 2.2.8. I could not compile.
But, the sysmouse is available since FreeBSD 2.2.
However, the names of function, macro and variable seem different.

To your help :-), I put mouse.tgz which contains console.h, mouse.h,
pcvt.4.gz, mouse.4.gz, sysmouse.4.gz, and pcvt.4.gz in FreeBSD 2.2.8 at
 http://www2u.biglobe.ne.jp/~hsaka/w3m/patch/mouse.tgz

# The test on my machine is difficult because some trouble occurred
